The short answer

JAKIM's notice for international manufacturing lists fifteen documents to send with the application form, including a cover letter, the applicant's Malaysian company registration, a manufacturing licence from the local authority, the Halal Executive's certificate and appointment letter, the Halal Assurance System documents, halal certificates or specifications for every raw material and processing aid, a process flow chart, packaging artwork, and the factory's layout plan and location map.

From JAKIM's own documents; each rule below carries its clause.

An overseas manufacturer applying for JAKIM halal certification sends a set of documents with the application form, and JAKIM's notice for international manufacturing lists them in its Table 1. JAKIM is Jabatan Kemajuan Islam Malaysia, the Department of Islamic Development Malaysia. This guide groups the fifteen documents by what they cover. Who may apply, and every route for a company outside Malaysia, is on JAKIM halal certification for overseas companies.
